Russ Abbott has created a Wiki page in order to collect
instances and examples of "Unintended Consequences".
An interesting topic, sometimes funny, and certainly
related to "emergence" and emergent properties.

I would say the most common form of unintended consequences
and side effects can be found in medicine. Do you agree ?
